چکیده

To the Editor: Alexander et al. (Aug. 23 issue)1 report findings from a prospective, multicenter gene-expression classifier validation study that raises several questions. For the 265 indeterminate fine-needle aspirates they tested, the geneexpression classifier had a sensitivity of 92%, a specificity of 52%, a positive predictive value of 47%, and a negative predictive value of 93%. Why the observed reduction in the performance of the gene-expression classifier, which was reported in their earlier study to have a sensitivity of 100% and a specificity of 73.3%?2 With a high negative predictive value and a low positive predictive value, the gene-expression classifier is useful as an adjunct to fine-needle aspiration biopsy for diagnosing indeterminate nodules as benign and potentially avoiding diagnostic thyroid surgery.3 However, the appropriate management of nodules diagnosed as “suspicious” (positive predictive value, 47%) according to the results of gene-expression classification is less obvious. Unfortunately, the gene-expression classifier does not have a positive predictive value that will allow us, as surgeons, to confidently plan an operation tailored to treat thyroid cancer (i.e., total thyroidectomy and central neck dissection). When considering the clinical and economic effect of a gene-expression classifier on the management of indeterminate nodules, avoiding thyroid surgery is not appropriate as the sole end point, and future studies must carefully evaluate the treatments and outcomes of all patients who undergo such testing.

Thyroid Ultrasound Features and Risk of Carcinoma: A Systematic Review and Meta-Analysis of Observational Studies

BACKGROUND Thyroid nodules are a common finding in the general population, and their detection is increasing with the widespread use of ultrasound (US). Thyroid cancer is found in 5-15% of cases depending on sex, age, and exposure to other risk factors. Some US parameters have been associated with increased risk of malignancy. However, no characteristic seems sufficiently reliable in isolation ...

Outcome of Subclassification of Indeterminate (Thy-3) Thyroid Cytology into Thy-3a and Thy-3f.

OBJECTIVES The British Thy system is a widely used classification system for reporting thyroid fine-needle aspiration (FNA) cytology. The Royal College of Pathologists in 2009 recommended the subdivision of the Thy-3 (indeterminate) category into Thy-3a (atypia) and Thy-3f (follicular neoplasm). Our objective was to examine the malignancy rates of Thy-3a and Thy-3f cases at our institution and ...

Real-time elastosonography: useful tool for refining the presurgical diagnosis in thyroid nodules with indeterminate or nondiagnostic cytology.

BACKGROUND Indeterminate and nondiagnostic patterns represent the main limitation of fine-needle aspiration (FNA) cytology of thyroid nodules, clinical and echographic features being poorly predictive of malignancy. The newly developed real-time ultrasound elastography (USE) has been previously applied to differentiate malignant from benign lesions. The aim of this study was to get further insi...

Contribution of scintigraphy and ultrasonography to the prediction of malignancy in thyroid nodules with indeterminate cytology Contribuição da cintilografia e ultrassonografia para a predição de malignidade em nódulos tireoidianos com citologia indeterminada

Objective: To evaluate the contribution of 131I scintigraphy and ultrasonography to the prediction of malignancy in thyroid nodules with indeterminate cytology in euthyroid patients. Subjects and methods: The sample consisted of 102 patients with thyroid nodules, submitted to FNAC and presenting an indeterminate cytological diagnosis (follicular neoplasm). Results: Malignancy was observed in 19...

Fine-needle aspiration cytology of thyroid nodules--how well are we doing?

OBJECTIVES To review the accuracy of fine-needle aspiration cytology in diagnosing non-toxic thyroid nodules and determine what factors are predictive of malignancy in patients with indeterminate cytology results. DESIGN Retrospective study. SETTING Regional hospital, Hong Kong. PATIENTS Patients with non-toxic thyroid nodules undergoing thyroidectomy from December 1999 to December 2003. ...

Q-Elastography in the Presurgical Diagnosis of Thyroid Nodules with Indeterminate Cytology

Quantitative ultrasound (US) elastography (Q-USE), able to evaluate tissue stiffness has been indicated as a new diagnostic tool to differentiate benign from malignant thyroid lesions. Aim of this prospective study, conducted at the Department of Surgical Sciences, of the "Sapienza" University of Rome, was to evaluate the diagnostic accuracy of Q-USE, compared with US parameters, in thyroid nod...
